dc.description.abstractHealth, United States, 2004 is the 28th report on the health status of the Nation and is submitted by the Secretary of the Department of Health and Human Services to the President and Congress of the United States in compliance with Section 308 of the Public Health Service Act. This report was compiled by the National Center for Health Statistics (NCHS), Centers for Disease Control and Prevention (CDC). The National Committee on Vital and Health Statistics served in a review capacity. The Health, United States series presents national trends in health statistics. Each report includes highlights, a chartbook, trend tables, extensive appendixes, and an index. An Executive Summary presents major findings.
