    Exploring Couple and Family Therapist Involvement in Social Justice Praxis

    As the nation becomes more diverse, multicultural competence and social justice are being increasingly recognized as essential components to effective therapy practice (Hays, 2020; Ratts et al., 2016; Vera & Speight, 2003). While some scholars in the field of Couple and Family Therapy (CFT) have urged the importance of infusing social justice into training and clinical practice for years (see Hardy, 2001; Knudson-Martin et al., 2019; McDowell et al., 2019; McGoldrick, 2007), this topic is understudied and underprioritized by the field at large. Recent CFT scholars also acknowledge the importance of advocacy as an accompaniment to therapy (J. M. Goodman et al., 2018, Jordan & Seponski, 2018a, 2018b). Counseling and social work fields have prioritized social justice advocacy and codified it into mission statements and ethical codes (Ratts et al., 2016; Ratts & Greenleaf, 2018; Toporek & Daniels, 2018). Although CFTs are trained systemically, and may be enacting micro-level advocacy intervention in the therapy room, they do not always view themselves as advocates or enact macro-level advocacy interventions (J. M. Goodman et al., 2018; Holyoak et al., 2020; Jordan & Seponski, 2018b). This study utilized a sequential transformative mixed methods design to assess multicultural competence, social justice commitment and self-efficacy, and advocacy competence in a nationally representative sample of CFTs (n = 101) using survey methods. A subsample of 22 participants were interviewed to further explore their practices as multiculturally competent and socially just clinicians. Three complementary frameworks were utilized to ground the study: The Multicultural and Social Justice Counseling Competencies (MSJCC), critical consciousness, and Public Health Critical Race praxis. Overall, multicultural competence, social justice commitment, and social justice self-efficacy scores were high in this sample, while advocacy competence scores were lower. Results showed that identifying as Black or African American and completing additional training in multicultural competence and social justice were associated with multicultural competence. Results also showed that working in an agency setting vs. other settings was associated with lower levels of multicultural competence. Results showed that identifying as female compared to male, having a higher level of oppression, a higher level of civic engagement, and more additional training in multicultural competence were all associated with social justice commitment. Results showed that being older, completing more additional training, and having a higher level of oppression were all associated with higher levels of social justice self-efficacy. Finally, results showed that identifying as non-binary compared to male, completing more hours of additional training, and experiencing higher levels of oppression were all associated with advocacy competence. Additionally, receiving more post-graduate hours of training in multicultural competence, social justice, and advocacy competence was associated with higher multicultural competence, social justice, and advocacy competence. Qualitative findings revealed ways in which CFTs developed and embodied socially just clinical practice and explored recommendations for training.

    Parental Health Literacy, Empowerment, and Advocacy in the Context of Food Allergies Management in Schools

    Background: Health literacy, empowerment, and advocacy may be important for parents when they communicate with schools related management of their child’s food allergies. Understanding prevention and emergency management of life-threatening food allergies may require high levels of health literacy and may be overwhelming to parents. Yet, parents are often the drivers of school food allergy safety practices. Mixed evidence supports the relationships among communicative health literacy, critical health literacy, and empowerment in chronic disease management. Objective: This cross-sectional study examines the relationships among parental health literacy, particularly communicative and critical health literacy; empowerment; and advocacy in the context of food allergies management in elementary schools. Methods: Parents of children with food allergies were recruited through food allergy organizations to complete an anonymous 20-minute online survey. Measurements of parental health literacy, empowerment, and advocacy were adapted from validated scales or the literature and refined through pre-testing and pilot-testing. Results: Participants (N=313) were predominantly white, college-educated mothers with moderately high food allergy knowledge, health literacy, and empowerment. Their children were allergic to an average of three food allergens and nearly half had asthma. Parents who scored at the highest levels on measures of communicative health literacy, critical health literacy, and empowerment engaged in advocacy behaviors perceived to be more effective than parents who scored at the lowest levels. However, this statistical difference may not represent a clinically significant difference. Communicative and critical health literacy were not more strongly associated with advocacy than functional health literacy. Empowerment and quality of the parents’ relationship with the school were the strongest predictors of the parents’ perceived effectiveness of advocacy efforts. The relationship between parental health literacy and advocacy was mediated by empowerment with a moderate effect size, but reverse causality between health literacy and empowerment could not be completely ruled out. Conclusions: Parental health literacy may impact the effectiveness of advocacy efforts for safe food allergies practices in schools, with parental empowerment possibly mediating the relationship between health literacy and advocacy. Longitudinal studies with diverse samples should verify findings. Health professionals should encourage parents to build good relationships with school personnel and help to empower families when educating them about food allergies management.

    Mental Health Advocates as Cultural Intermediaries: A Sociocultural Perspective of Advocacy and Legitimacy

    This study sought to understand how advocacy as a public relations activity can give voice to marginalized publics and how/if mental health advocates perceive their advocacy work as influencing culture as it relates to mental health. This study incorporated the theoretical frameworks of fully functioning society theory (FFST) and the circuit of culture. Additionally, this study investigated the concepts of advocacy, legitimacy, cultural intermediation and discourses as they entangle with and within FFST and the circuit of culture. The juxtaposition of these theories helped to uncover both culturally situated best practices of advocacy and interrogate the frames that underpin the rhetorical ideals of a fully functioning society. This study employed qualitative, in-depth interviews with 38 mental health advocates who communicated a variety of perspectives about mental health and illness. Some themes that emerged include: advocacy as both education and empowerment, legitimacy as authority that can derive from both lived and learned experience, the multiple subcultures within the field of mental health advocacy, and the variety of mental health discourses. Furthermore, two overlapping, but distinct missions of advocacy exist including 1) general mental health for all of society and 2) advocacy for people who have experienced significant challenges to daily life and/or harm (e.g., prejudice, discrimination) because of a psychiatric diagnosis. This dissertation extended FFST and the circuit of culture to present a culturally embedded conceptual model of advocacy and theoretical propositions to help guide future theory building. The theoretical propositions outline how advocacy is a vehicle for voice to change status quos, how dysfunction and marginality are parallel within FFST, and how legitimation and cultural intermediation align in the context of culturally situated responsible advocacy. The findings also contributed to the existing theories by applying those theories to a specific context of mental health advocacy, which illuminated the importance of questioning normalized values within FFST and approaching intermediation with reflectiveness. This research considers the consequences of advocacy for people with lived experiences and situates advocacy within social justice contexts to inch closer towards the ideals of FFST.

    Charles Fowler and His Vision for Music Education: An Introduction and Selected Writings From 1964 to 1989

    Charles Fowler, eminent advocate for arts education, devoted his career to the idea that music was critical to the development of young people and could positively impact schooling and society. During his 45-year career, he served in many roles: as teacher, supervisor, professor, scholar, author, editor, consultant, and advocate. Although his contributions are prolific, this research represents the first full-scale study that considers his work as an entire body. The purpose of this dissertation is to introduce Fowler as a significant figure in the field of music education, codify the major periods of his career, identify important writings, contextualize them within their times, and review them according to his vision for music education. Utilizing historical method and content analysis, several thousand documents were examined from the Charles Fowler Papers archived at the University of Maryland, College Park. Following an introduction to Fowler and his work, four periods of his career are presented, with the two middle periods, 1964 to 1973 and 1974 to 1989 serving as the focus of the study. Selected works were chosen based on their relevance to important events in Fowler's life and their relationship to his philosophy and viewpoint. The works were analyzed and contextualized by using primary source documents, foundational texts in music education, Fowler's own commentary, and interviews with established scholars and colleagues who knew him and respected his work. Finally, these writings traced the development of Fowler's vision which advocated music education can serve as an agent of social change. Findings reveal that Fowler's initial vision was based on the seven reconstructionist objectives he outlined in his 1964 dissertation. Based on these objectives, fifteen broad themes emerged in his writings during the period of 1964 to 1989. The themes elaborate on Fowler's vision for music education and its value to society, and relate to core concepts of reform, democracy, creativity, advocacy, and social change. It is hoped that this study will serve as a catalyst to encourage others to continue research into the life and career of Charles Fowler, along with further writing about reform and pragmatic change within the music education profession.

    Relationship Management and Member Retention: A Case Study of an Advocacy Organization

    A case study of a grassroots advocacy organization was conducted to test and expand relationship theory and to explore perceptions about the relationship between members and the organization. The case study included interviews with 39 staff members at national, state, and affiliate levels; 58 members; and 5 former members, for a total of 102 participants. Additional methods included 49 hours of participant observation and an examination of both internal and external documents. The primary relationship type between the organization and its members was communal, and strategies were presented to cultivate communal relationships. This study empirically justified the critic's perspective for classifying relationship types due to one case in which three relationship types emerged, depending on whether the former member's, affiliate staff member's, or my interpretation was used, which also resulted in a new relationship type. Due to these differences in perceptions, this study used the terms intended and perceived when identifying relationship types, which is a clarification for future studies to use. Cultivation strategies were organized in a new way by classifying them as either organizational management strategies or as interpersonal strategies. This study also discussed cultivation strategies by characterizing some as particularly important to either the early stage of the relationship or to the mature stage of it. Several new cultivation strategies were presented, such as priming, problem parking, and insulation. This study also opened a new area for relationship theory through a conceptualization and exploration of relationship stresses. This category is organized by stresses that are internal to the organization and those that are external to it. Examples of relationship stresses include the emotion tax, relationship speeding, and relationship stalling. Cultivation strategies are suggested for mitigating relationship stresses. In addition, this study produced significant insights outside of the research questions by identifying new relationship outcomes, such as co-production, and by claiming capacity to be a higher goal than survival for systems theory. Furthermore, this study clarified the difference between an advocacy and an activist organization. This study also provided rich insights for public relations practitioners, such as presenting strategies to diversify an organization's membership.
